How to Complete the Work Permit Request Form

Student

Neatly print your name, address, telephone number, social security number, and date of birth.

Parent/Guardian

Please put your signature on the last line of the form where indicated. If the student is over 18 years old, this is NOT necessary.

 Employer/Supervisor

  1. Complete name, address, and phone number of business.
  2. Sign on the line indicated "Supervisor's Signature."
  3. Your new employee will return an "official permit" to you within two days. If you do not receive it, be sure to ask the student as they are not allowed to work without the permit.

Email completed work permit to prenireyhanian@burbankusd.org. You will receive the permit back within 24-48 hours.

Burbank Unified School District Guidelines

Students 12–13 years old may be employed only during school holidays and vacations (usually construed to include weekends). May never be employed on any school day, either before, during, or after school. [EC 49111] 
 
Maximum Hours 
Daily: 8 hours.
Weekly: 40 hours.
Spread of Hours 7am - 7pm (- 9pm June 1 through Labor Day).

1. An employer shall not employ a minor 15 years of age or younger for more than eight hours in one day of 24 hours, or more than 40 hours in one week, or before 7 a.m. or after 7 p.m., except that from June 1 through Labor Day, a minor 15 years of age or younger may be employed for the hours authorized by this section until 9:00pm.

2. Notwithstanding paragraph (1), while school is in session, an employer shall not employ a minor 14 or 15 years of age for more than three hours in any school day, nor more than 18 hours in any week, nor during school hours, except that a minor enrolled in and employed pursuant to a school-supervised and school-administered work experience and career exploration program may be employed for no more than 23 hours, any portion of which may be during school hours.

3. Must maintain a 2.0 GPA. Must have attended at least 95% of the school year at the time that student applies for a work permit. Students who have had one or more suspensions may not be eligible for a work permit. 
 
4. Records will be reviewed regularly and work permits will be revoked for students who do not meet the 2.0 GPA and acceptable attendance requirement.

PLEASE NOTE: All minors under the age of 18 (including minors employed by parents) must have a work permit. Work permits must be renewed at the start of each school year or at the time a student obtains a new job.
